JOB SUMMARY:
The Registered Nurse/CPN functions in the physician office or ambulatory care practice to provide nursing services in the office setting and/or via telephone assessment and triage. The RN maintains clinical and professional competency as appropriate to the ages, developmental stages, and special needs of the patients served. The RN provides supervision and leadership as appropriate within the clinical environment.
ESSENTIAL FUNCTIONS:
1. Model CHS core values of caring, commitment, integrity and teamwork to internal and external (i.e. patient, family, physician and team members) customers.
2. Demonstrate the knowledge and skills necessary to provide care or arrange for the provision of appropriate care, for newborn, pediatric, adolescent, adult, and/or geriatric population of patients, as pertinent to the scope of the office setting.
3. Perform telephone triage. Obtains clinically appropriate data and symptoms from the patient to advise and instruct patient according to established protocols and/or orders from the physician.
4. Perform clinical functions to include but not limited to:
• Medication administration, including injections/immunizations;
• Administration of intravenous fluids under the direction of the physician;
• Completion of CLIA approved waived laboratory tests and practice-specific diagnostic testing; if applicable.
• Vital signs and patient assessment prior to physician visit;
• Nurse visits under the supervision of the physician;
• Observes, records and reports patients condition and reaction to the physician following procedures, medication administration or treatments;
• Assists providers during procedures and examination and performs instrument and room clean up in accordance with OSHA standards.
5. Utilize standard precautions and complies with OSHA/CLIA regulations in completing job assignments. Educate other staff regarding these regulations.
6. May assist with patient inquiries for prescription refill and scheduling appointments for patient referrals and obtaining authorizations. Notifies patient as directed by the provider.
7. Provide patient education as specific to the patient’s needs, including but not limited to disease process, medication use and contraindications, collection of samples and follow up needs.
8. Responsible for monitoring inventory and maintaining approved stock medications, narcotics and emergency medications/equipment specific to the practice setting.
9. Serves in a leadership role within the clinical environment and as clinical resource for the office manager and other nonclinical stuff.
MARGINAL FUNCTIONS:
Participate in quality assurance programs, attend safety and staff meetings and in-services. Aid in maintaining stock supply items, inventory management, restocking exam rooms, emergency equipment, etc. Complete lab requisitions and prepare specimens for pick-up. Assist provider with appropriate administrative tasks. Assist with referrals, including obtaining authorization. Venipuncture. Performs other duties as assigned.
PHYSICAL REQUIREMENTS:
Work consistently requires walking, standing, sitting, lilting, reaching, bending, stooping, pushing, and pulling. Must be able to lift and support the weight of 50 pounds in handling patients, medical equipment, and supplies. Possible exposure to communicable diseases. Must speak, read and write basic English fluently. Intact senses of sight, hearing, smell and touch. Manual finger dexterity. Critical thinking and ability to concentrate. Must be able to respond quickly to changes in patient and/or unit conditions.

Demonstrates knowledge of the principles of growth and development and possesses the ability to respond to age specific issues and data reflective of the patient’s status. Demonstrates the knowledge and skills necessary to provide care for the following age groups:

Nurse to patient ratios to expect 1:5-6
On occasion, staff will be asked to float to mid shift the other location if the location is in an urgent need
Medical surgical/telemetry (MST) assignments will be taken in one of our 12 medical/telemetry units.
Assignments are based on unit needs and are monitored throughout the day by our Staffing Office/Nursing Supervisors.
At times both UHS staff and our travel nurses will be asked to float to two different units during one shift. Full charting to be completed every 8 hours – expectation is that staff will be required to do a full documentation on each unit staff are assigned to during their 12 hour shift. We realize this is not ideal, but to provide safe staffing there will be times this is needed.
UHS does not have a holds/observation unit-
If census is high patients may be held in our emergency departments.
These patents have been admitted and are under the care of an attending physician.
To care for these patients, float pool staff go to the emergency department and treat these patients as if they are on an inpatient unit.

They do approx. 50-60 visits a day
Infants to End of Life, CHF, NSTEMI, abdominal pain, minor trauma (MVA), ER holds. Major traumas are stabilized and life flighted out.
Heavy psych population – ETOH withdrawal; IVDA
Patient Ratios:
RN – 1:4-5; on night shift there are times that depending on Census, there may be only 2 RNs for the night shift
Tech – 2 per shift when fully staffed (one acts as HUC, the other will support the RN staff with CNA duties)
Required Certifications:
RN: BLS, ACLS, PALS, TNCC
Tech: BLS, EKG certification (preferred)
Skills required:
RN:
Travelers do not necessarily need charge experience but need to be willing to be trained and act as charge as often as needed. Please decline traveler if they’re unwilling to do this.
General ED skills, PIV, trauma stabilization, procedural sedation is a MUST, cardiac rhythm interpretation
Tech:
50% of the time they will work as Unit Secretary and the other 50% will be as CNA performing patient care duties to assist the nursing staff:
Vitals (inpatient and possibly outpatient for patients waiting in their cars), blood glucose checks, ADL’s, transporting patients, lab runs, any other task as assigned; experience with EKG’s (preferred)
Support on the Unit:
Tech – 2 per shift when fully staffed (one acts as HUC, other will assist with patient care)

POSITION DESCRIPTION: MAIN EMERGENCY ROOM- Min of 3 years ED experience required. Must have over 1 years of LARGE LEVEL 1 TRAUMA ER Experience. Every other weekend and every other holiday required. No call. ICU Skills is also helpful as many patients require ventilator, pressure monitoring lines and other procedures. Work history in packet must show # of beds, Trauma Level and EMR or they will be dropped. ACLS and BLS required, must be in packet at time of submission. Preferred PALS and NRP. Must have EMR: EPIC experience; Scrubs: pink or navy, Ratio 1:1 - 1:3. Must have active MD/Compact license in packet at time of submission, no temp licenses allowed. PROOF OF COVID VAX REQUIRED AT TIME OF SUBMISSION. IVS to Interview and Manager to Offer. ***UMMs will not accept professionals that have attended one of these schools identified by the DOJ. UHS Hospitals Medical Surgical/Telemetry Traveler Float Pool Guidelines

What to expect when joining our traveler float pool-
Staff will be asked to float between the Johnson City(33-57 Harrison St, Johnson City, NY 13790) and Binghamton hospital(10-42 Mitchell Ave, Binghamton, NY 13903) campuses.
Staff will float to other Tele and Med Surg units.
Nurse to patient ratios to expect 1:5-6
On occasion, staff will be asked to float to mid shift the other location if the location is in an urgent need
Medical surgical/telemetry (MST) assignments will be taken in one of our 12 medical/telemetry units.
Assignments are based on unit needs and are monitored throughout the day by our Staffing Office/Nursing Supervisors.
At times both UHS staff and our travel nurses will be asked to float to two different units during one shift. Full charting to be completed every 8 hours – expectation is that staff will be required to do a full documentation on each unit staff are assigned to during their 12 hour shift. We realize this is not ideal, but to provide safe staffing there will be times this is needed.

Critical care assignments will be taken in our 14 bed ICU, 16 bed CVICU, 6 bed medical ICU (Binghamton campus), 6 bed neuro and 5 bed surgical step-down suites
While working in our critical care areas nurses maintain a 1:2/ 1:3 acuity based nurse to patient ratio.
Stepdown ratios are 1:4
Floating Outside of Critical Care
When census or acuity drops, staff may be asked to float outside of the Critical Care setting.
If there is NOT an appropriate Critical Care Assignment, staff are assigned to a telemetry/medical units where a 1:4 ratio will be maintained.

Patient Types:
They do approx. 50-60 visits a day
Infants to End of Life, CHF, NSTEMI, abdominal pain, minor trauma (MVA), ER holds. Major traumas are stabilized and life flighted out.
Heavy psych population – ETOH withdrawal; IVDA
Patient Ratios:
RN – 1:4-5; on night shift there are times that depending on Census, there may be only 2 RNs for the night shift
Tech – 2 per shift when fully staffed (one acts as HUC, the other will support the RN staff with CNA duties)
Required Certifications:
RN: BLS, ACLS, PALS, TNCC
Tech: BLS, EKG certification (preferred)
Skills required:
RN:
Travelers do not necessarily need charge experience but need to be willing to be trained and act as charge as often as needed. Please decline traveler if they’re unwilling to do this.
General ED skills, PIV, trauma stabilization, procedural sedation is a MUST, cardiac rhythm interpretation
Tech:
50% of the time they will work as Unit Secretary and the other 50% will be as CNA performing patient care duties to assist the nursing staff:
Vitals (inpatient and possibly outpatient for patients waiting in their cars), blood glucose checks, ADL’s, transporting patients, lab runs, any other task as assigned; experience with EKG’s (preferred)
Support on the Unit:
Tech – 2 per shift when fully staffed (one acts as HUC, other will assist with patient care)
HUC, 24-7 - also does telemetry monitoring
Charge nurse – may have patients, depending on staffing
