Epic Travel Staffing is hiring a Travel – Case Manager/Outpatient Clinic

Shift: Variable, 8’s/10’s, 40 hours per week

Length: 16 weeks

Weekend REQ: various, but no more than 1-Saturday per month

Requirements:

  • OR License
  • 2 years experience, within the last three years, experience working in direct patient care
  • Within the last 3 years, experience in working an ambulatory care setting including telephone triage
  • BLS

Nurses perform patient visits, telephone triage and advice, and manage an in-basket workload in EPIC where pts. are reviewed for Care Management, Care Coordination, follow up for discharge planning, and triage/advise for medical conditions. Care Coordination and Disease Management are a high priority aspect of the job. Nurses work in pods within a care team of Providers, Pharmacists, and Behavioral Health clinicians.
