Epic Travel Staffing is hiring a Travel RN – Case Manager
Shift: Days/ 8a-4:30p, 8×5, 40 hrs per week, Holidays as needed, No On-Call
Length: 13 Weeks
2 weekend shifts per contract
Requirements:
Position/Specialty: Nurse Case Manager
Ratios: 1:30
Start date: Start date will be 2-3 weeks from offer
Shift: Will work 5-8 hour days, 8am-4:30pm with holidays as needed
Weekend REQ: 2 weekend shifts per contract
Is on-call REQ? No
Will clinician float within scope to meet facility needs? No
Required/Preferred
Years of experience REQ: 3 years of RN experience
First-timers accepted: No- Must have Travel Experience
Certs/Lic REQ: BLS
Education REQ: BSN
Certs Preferred: Case Management certification
Experience Preferred: Prior leadership experience and Epic Experience
Looking for a Nurse Case Manager (NCM) I reports to the Manager of Case Management and Care Transitions. Under guidance of the Nurse Case Manager Team Leader, the Nurse Case Manager I functions as a member of the clinical team. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system. The Nurse Case Manager I serves as a clinical expert related to discharge planning, resource utilization, and case management.
