Registered Nurse – Home Health

The Home Care RN Team Nurse is a registered nurse who applies nursing knowledge and clinical skills using the nursing process to provide high-quality, individualized, safe care to patients in their home environment The RN Team Nurse utilizes nursing assessment skills, critical thinking, problem-solving, and communication in providing home health services to patients. The Home Care RN Team Nurse works closely with the case manager, team members, and other health care providers to assess, plan implement and evaluate patient care with minimal direct supervision.
