Travel Medsurg/charge with possible float to ER RN is needed to start in a facility in North Dakota. ND or compact license is required. 2 years of experience is required. Certifications required are ACLS, BLS, and TNCC. PALS is preferred. Shifts are rotating D/N -7-7, every other weekend or every 3rd weekend plus rotating holidays. Block scheduling is a possible option. Medsurg—25 beds but 16 of them are long term swing bed patients—The other 9 beds are inpatients, outpatients, and skilled swing bed. Nurse to Patient Ratios in each of your units – Telemetry, Medsurg—3 nurses staffed for approximately 19 to 20 patients (one nurse with several CNAs takes care of the 10- 14 LTC swing beds. Telemetry - Do you have telemetry on your unit or in your department? Yes we have 4 patient telemetries What brand are the monitors? Philips Do nurses have to read & interpret strips? Yes Do you have monitor techs? NO Does you unit titrate any drips? Med surg—pain gtts for end of life, Heparin—occasionally, Diltiazem, Insulin, Nitro, Treatment gtts like Remicade, IgG, etc., TXA, tPa, fluids, Blood, platelets, etc. If so which ones? Are they set rate? Yes, mostly are set in a smart pump. Vents –Bipap, Cpap, or High Flow? No, just a portable ventilator for transport. Patient Types:
Discharge Ready Unit
A variety of patients are cared for here who are awaiting placement in the community
Heavy behavioral/psych on top of LTC/SNF. Security is present on the unit due to aggressive patients
Typically, non-acute – without the need for special monitoring or IV access
Occasionally, patients are admitted for IV antibiotics only, but they do not receive frequent doses
Mobility of patients range from independent transfer to Hoyer/2 assist
Patients are cared for under a nursing home model with focus on fall prevention, skin integrity and activities
Supported by activities coordinator, CNAs and expert care management staff
Nursing care is less than acute level of care i.e. vital signs are once a day; full assessment is weekly
Patient Ratios: Team Nursing Approach
RN: Days and Nights - up to 1:9; 4 RN’s + 1 CRN (unassigned)
CNA: Days and Nights - up to 1:8; goal 1:6-7
LPN: Days and Nights – up to 1:9 – future implementation – no LPN’s currently

Skills required:
Experience caring for Geriatrics (ideal candidate would have SNF or LTAC experience)
RN: Charge experience, dressing changes, IV antibiotic administration (most patients will not have IV access), enteral feedings
LPN: Plan of cares, medication administration (oral, IV fluids/IV antibiotics; no IV pushes or initiation of high-risk meds/blood products – can monitor high risk meds after RN initiation), inserting peripheral IV (with met competency), dressing changes, central line flushes, wound care, vital signs, ADLs
Daily Responsibilities:
RN:
0600 or 1800 med pass for most meds, there may be meds due at other times
Dressing changes
Straight caths (some patients straight cath themselves)
Charting:
Med scanning
Daily nursing notes to include the following (1 per shift per patient – 10 total):
Activities they participated in during that shift
Behavioral Notes
Overall impressions
VS as ordered (most often weekly or PRN if they have BP issues)
CNA:
Daily ADL charting
Meal intakes
Passing trays
Activities
Ambulation
Getting patients to/from activities and meals
Weekly showers/shower schedule
LPN:

Will accept a first-time traveler
Permanent staff must wait 2 years before returning as a traveler
Travelers cannot live within a 75-mile radius of the facility
Patient Types:
Medical Type Patients: CHF, Diabetes, Dialysis, COPD, ETOH, IVDA
Telemetry – remote monitoring with tele tech 24/7
Will not have to read or verify strips. Tele tech will notify RN of rhythm changes
General Surgical Patients
Post-operative and observation
Patient Ratios:
1:5-6, can flex up to 7 in surge times (do not go above 1:7) -- working on getting this down by hiring more staff
Required Certifications:
BLS
CPI or MOAB – must have prior to start
ACLS (preferred)
Skills required:
IV insertion, trach management, chest tube management, NG Tube placement and management, feeding tubes, foley placement and management, wound care, dressing changes, central line management
Cardiac rhythm interpretation (preferred)
Support on the Unit:
CNA (Ratio 1:8-9, can be as high as 1:12 due to)
Unit Secretary – Day shift
Telemetry suite with tele tech 24/7
Charge nurse takes a patient load
Hospitalist, RT in house 24/7 (RT on unit)
Phlebotomy does most peripheral draws, nurses will have to draw all central lines

Orientation:
1 Day of general orientation (get badge, learning modules)
If general orientation is only part of day, and the remainder of the day will be used for on unit orientation

Floating:
To any med-surg unit and holding/overflow unit
Scheduling:
Every other weekend expectation, rarely back to back weekends
Shift Times: Day: 0700 – 1930; Night: 1900-0730
No self-scheduling
RTO requests: Clinical team to approve up to 7 days off. Need approval from unit Manager for greater than 7 days off.
Holidays: will be assigned a holiday in rotation – request RTO approval for holidays
