2 Pediatric Beds (pediatric admissions have grown greatly in the past year, 20+ admissions age 0-1 in the past year, mostly respiratory)

Patient Types:
L&D: They will deliver the mom and then either:
Transfer care to the post-partum nurse if there is another laboring mom
Transition to taking care of the couplet postpartum
May have to catch the baby with the pediatrician for C-Sections if nursery nurse is unable to- Most often 2 WCH RNs attend c sections, at risk deliveries will be 1 RN and 1 pediatrician
Post-Partum: Take care of couplets, need to be comfortable with both Mom and baby
Nursery: Will catch the baby with the pediatrician for C-Sections or a WCH RN
C-Sections:
At night, the L&D nurse will take the baby back to the unit after delivery. All stable spinal c-sections recover in the department, patients go directly from OR back to LDRP room and WCH RN performs PACU recovery.
During the day, the mother and the baby stay together in the recovery room
Pediatrics (not a deal breaker if they do not take care of peds)
Appys, respiratory issues (bronchiolitis), GI/ dehydration
Peds in need of IV ABX and/or fluid resuscitation
Any critical peds gets shipped to EMMC

Mother and baby live in model. They don’t take the babies to the nursey unless circumstances require it. Comfortability with couplet care/newborn care is a must. Experience with lactation support is very important.
Needs to be able to resuscitate/practice NRP
They may have magnesium gtts occasionally but no insulin gtts
Support on the Unit:
1 L&D nurse
1 Nursery nurse
1 nurse that is either postpartum, LD or nursery nurse (all three or a mix of the three)
Sometimes the 3rd nurse is on call if census is low
3 Obstetricians and 4 Midwives
Pediatrician team on call (no more than 15 mins out)
RT on call at night, on the unit during the day
If there is a suspicion that it is going to be a challenging birth, they call EMMC, they send a team up to transport the baby. Usually within 3h of birth, the baby is transferred
Anesthesia is on call at night (physically available within 15 mins). Some choose to stay if they have an active epidural, but they are not required to.
Charting System: Cerner and FetaLink
Floating: if zero labor and delivery census, may be asked to assist with med surg care of overflow patients on the unit, but not take an assignment just be support for other WCH RNs

Two days of hospital orientation
2 days on unit

This unit resides inside of pre op and has its own rooms. 1-5 and 6-10. – All private rooms for the overnight stay
Nurse staffing ratio 1:3-5 patients per ASPAN Extended Care guidelines, with one CNA for up to 10 patients
Hours: Mon morning – later start – patients typically arrive between 10am-1pm - and open to Saturday until 12 noon-1pm. Closed Sundays.
Phase 3 Recovery: 10 recovery rooms – no more than 10 cases per day. Patients will be recovered through phase2 in main PACU, and PACU RN will give report to accepting phase 3 nurse.
In Main PACU (will work if census is low) –
37 recovery rooms
12 for outpatients, 11 for inpatients; 6 used for overflow, staging, and inpt holds
Approx. 50 cases a day
24/7 operating hours. Open weekends and holidays.

Simple to complex surgeries needing MAC anesthesia to General, and spinals.
Occasionally will be ICU and ventilated/ intubated – not a required skillset for Phase 3 RNs

Orthopedics: Total hip/knee and shoulder arthroplasties
OB/GYN: urinary slings, hysterectomies, BSOs, post C-section Maternity
General/GI: breast biopsies, bowel resections, lobectomies, colostomies, hernias, carpal tunnel repair
Bariatric: Gastric bypass, gastric sleeves
GU: cystos, laser lithotripsies, whipples, nephrectomies
Neuro: Spinal surgeries, Craniotomies, Shunts
Endo: EGDs, IPR (interventional radiology/cardiology)
Pediatric cases: dental hygiene, eye surgery, pain pumps, ports, hemorrhoidectomies
Mandatory skills:
Strong knowledge of post op total joint care and getting patient up and ambulating

Phase 3 RN’s will float to Main PreOp/PACU and assist if census is low
